Start the quest "Arms of Atonement" from Elpenor then find the first piece, Gauntlets of the Fallen then find the rest of the fallen warriors to complete the set. Open the video guide above and locations are time stamped for your convenience. To get the items you need, you have to kill the fallen warriors who has that piece listed below.

Gauntlets of the Fallen 0:49 - meet and get it from Elpenor
Helmet of the Fallen 2:27 - kill the Poisoner of Athens
Chestplate of the Fallen 6:27 - kill Swordfish
Waistband of the Fallen 9:29 - kill Epiktetos
Boots of the Fallen 12:21 - kill Deianeira

Upon completion, you will also earn the "Death Walker" achievement
